Clinical Application Project/Community Health Project Part I

Purpose: The purpose of this clinical application project is for students to assess the health care needs of a population within their community. This is an independent assignment.

Task: Working within the community, students will identify a community based coalition, school, civic organization, occupational health setting or other practice site for this clinical experience and work with this community to identify and prioritize a community diagnosis and develop a plan to address it. Students are encouraged to begin this process early and identify a mentor within the chosen clinical/practicum site. Students will complete the Organizational Mentor Form to course faculty for review and feedback before beginning this assignment. Students are expected to use adequate and appropriate resources to support your discussion.

Assessment is the first step in the nursing process. Assessment of the community will involve statistical analysis as well as observations. Local and State Health Departments and the Centers for Disease Control (CDC) will offer statistical data to guide this project. Refer to Healthy People 2020 and include social determinants of health as you begin to assess your community and identify an population of the community that has a need and may benefit from intervention.

Once you identify the population, continue the nursing process with determining diagnoses, prioritizing diagnoses and developing interventions to address the identified need.

Refer to Chapter 9 in the course text for guidance with planning for change.

Literature Review

To begin the discussion review 5 or more current nursing research articles and research from other professions. This literature should be used to support your discussion of the characteristics, problems/needs of population. (A minimum of 5 total resources is required) Literature reviewed but not cited within the discussion should appear on reference page.

Assessment

Define your identified population and rationale for selection. Discuss methods for gaining entry to the community and include any research or tools utilized

Discuss the characteristics of population. Use data sources to define socio-demographic characteristics. Discuss actual and perceived health status. Identify both internal and external influences affecting population health

Compare/Contrast your chosen population with similar populations, the community, the state and/or the nation. Provide specific examples and/or current statistics.

Nursing Diagnoses (plural) should accurately reflect the needs of your population Support your Nursing Diagnoses with rationale and theory support and prioritization applied to population. Include comparative analysis and interpretation of data collection and current research relative to health problems/needs.

Compare/Contrast- Compare and contrast target population with similar populations, the community, the state and/or the nation. Provide specific examples and/or current

statistics.

Planning

Health Planning/Needs- Select and discuss one priority Nursing Diagnosis which needs intervention. Provide specific, measurable objectives.

Alternative Interventions- Include description of alternative interventions necessary to fulfill objectives. Discuss either existing or developing resources.

Format

Appropriate APA formatting using the APA 6th edition guidelines, correct grammar and spelling are expected.

This discussion should not exceed 6-8 pages excluding title page and reference page, should be doubled spaced and follow APA 6th edition guidelines. Proper grammar and syntax is expected

All statements should be supported with professional literature. A minimum of 5 sources of nursing research are required. Literature reviewed but not cited should appear on reference page.

Use of material from websites should be judicious ** Keep in mind that when you use/cite a source you are identifying the author as an expert worthy of being cited. Lay person web sites and blogs are generally not appropriate for use in professional discussions.

Clinical Application Project/Community Health Project Part II

Purpose: The purpose of this clinical application project is to allow students the opportunity to refine the planning process and implement and evaluate the interventions designed to address the health needs of the chosen population within their community. This is an independent assignment.

Task: As you continue working with the population identified in Part I of this project you will refine the plan and continue with the implementation and evaluation phases of the nursing process as it is applied to your population. To be successful with this project you will implement your interventions and evaluate the outcomes.

There is an 8 page maximum limit on this paper.

Planning

Nursing Diagnoses- Discuss health problems identified in the assessment phase (Part I) with rationale for selection with resources provided. Please use traditional nursing diagnosis format. Please make sure your diagnoses are community oriented and incorporate the community assessment data.

Priority diagnosis- Identify a priority nursing diagnosis with sound rationale and EBP support. Identify an overall goal and specific and measurable objectives.

Interventions- Describe interventions necessary to accomplish objectives. Include consideration of interventions at each systems level (Review Text p 155) where appropriate. Select and validate intervention(s) with highest probability of success. Include use of existing or developed resources.

Intervention

Planned Interventions- Identify one intervention at appropriate level (primary, secondary or tertiary) and support intervention with EBP.

Literature -Please identify 5 acceptable resources to support your planned interventions.

Barriers-Identify barriers and discuss remediation.

Evaluation

Planning- Develop an evaluation plan that addresses achievement of objectives as stated to include processes and rationales.

Limitations- Discuss limitations relative to the evaluation process or tool used and suggestions for resolution.

Recommendations– Address recommendations for further action based upon evaluation and communicate to appropriate individuals.

Implications- Discuss the implications for Community Health Nursing.
